Insurance Information

We accept most major insurance plans.

Appointments

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

Prescriptions & Renewals

Our office is normally open for telephone calls from 10:00 a.m. to 4:30 p.m., Monday through Friday. The doctor will not refill prescriptions outside of the normal working hours. Please check your medications on Thursday to make sure you will not run out over the weekend.

Preparing for Your Appointment

As a partner in our health care, it is important that you prepare for your appointment and bring the items necessary to make your appointment successful.

Plan to arrive at least 15 minutes early for your appointment to allow time for our registration process and history form completion. We also require a picture ID. We make every effort to respect your time.

What You Should Bring

  • Medical Records – If your visit is on a physician’s referral, you may need to bring medical records, x-rays, test results or other materials. Please bring these with you or arrange for them to be sent prior to your visit.
  • Personal Medical History – In order to provide the best care, please bring a record of all current prescriptions and non-prescription medication. We will also need specific information about your current injury, onset of symptoms and related history.
  • Insurance Information – As a courtesy to you, we are happy to bill your insurance carrier for you. In order to do this, we need a copy of your current insurance card. If you have no health insurance, please review the Financial Policy on page 2.
  • Referrals – A major point of confusion is the insurance referral. It is very important for you to understand your insurance plan. If you have questions about your insurance you should call the telephone number listed on the back of your insurance card. If your insurance requires a referral, you are responsible for making sure your primary care doctor has given you one prior to your visit. Even if your primary care doctor made the appointment, the referral may not have been entered. Please call your primary care doctor to insure that you have one. It is not the responsibility of our staff to get your referral.
  • Workers' Compensation Information (If applicable) – In order to file your workers’ compensation claim, we need the full name and mailing address of your worker’s compensation carrier, the date of the injury, claim number, and the address and phone number of your employer at the time of the injury. In addition, we need your Private HealthCare Insurance information.
  • Motor Vehicle Accident – In order to bill your motor vehicle insurance, we will need the name of your insurance company, address, telephone number, adjusters name, claim number and date of accident. We will also need your Private HealthCare Insurance information.
  • Payment – Please be prepared at the time of service to pay the office visit co-payment required by your insurance plan. Some services may not be covered by insurance and would require payment at the time of your visit. We accept cash, checks, and credit/debit cards (Visa and MasterCard only).
